ÉquiToît is studying the possibility to implement appartments at the Convergence Griffintown Project developed in partnership with the CIUSSS Centre-Sud de l’île-de-Montréal, Espace La Traversée and Bâtir son quartier.
The building, which would hold 272 apartments, would be built in Griffintown. Opening would be in 2022. Around 200 apartments would be for seniors and 15 to 20 adapted apartments would be reserved for our project, situated on 1 or 2 floors. The majority of apartments would be 3 ½ and 4 would be 4 ½ (to be confirmed).
This project would aimed at adults with an ISO-SMAF profile of 4, 6, or 9 and who require between 25 to 45h of services per week.
Services would be ensured by the CIUSSS Centre-Sud -de-l’île- de- Montréal. The offer of services will be specified at a later date by the CIUSSS in collaboration with ÉquiToît. Services will include;
an individualized service plan
coordination of by an onsite supervisor
services will be given by an certified ASSS, available onsite 24/7. This will include help for getting up/going to bed, personal hygiene, help for meals (serve, heat up, cut up…)
help for domestic activities: cleaning, laundry, change the bed, wash the dishes, put away groceries, take out the garbage, preparations of simple meals…
Individual needs for professional resources in home care(nursing, physiotherapy, psychosocial services,...) will be evaluated according to SAD criterias
