Le Monkland is located at 4400 West Hill in NDG. It is developed in partnership with the Société d’habitation de Montréal (SHDM) and the CIUSSS Centre-Ouest-de-l’île-de-Montréal (CIUSSS-CODIM). The building holds 100 apartments for autonomous people 55 years and over.
The project is made up of 7 residents who were chosen by a joint ÉquiToît-CIUSSS-CODIM selection committee. To be selected, candidates had to have an ISO-SMAF profile of 4, 6, or 9 and require no more than 35h of services per week.
Residents have signed a lease with the SHDM for their 3 ½ apartments. The adaptations are underway with the SHQ and the City of montreal, through the PAD program.
The services will be ensured by the CIUSSS-CODIM, through a home care agency. Eventually, someone will be present day/evening/night to meet the needs of residents. An extra 20h/week is planned for rush hours. The maximum number of hours of services given by the CIUSSS is 5h/day or 35h/week.
Services provided include:
an individualized service plan
coordination of services by an onsite supervisor
services will be given by a certified ASSS, available onsite 24/7. This will include help for getting up/going to bed, personal hygiene, help for simple meals (including toasts, coffee)
help for domestic activities: cleaning, laundry, change the bed, wash the dishes, put away groceries, take out the garbage, ..
There is no meal preparation included. A catering service or something similar could be set up. The cost of meals will be assumed by the residents according to their use.

CONVERGENCE GRIFFINTOWN PROJECT (still in discussion)
ÉquiToît is studying the possibility to implement appartments at the Convergence Griffintown Project developed in partnership with the CIUSSS Centre-Sud de l’île-de-Montréal, Espace La Traversée and Bâtir son quartier.
The building, which would hold 272 apartments, would be built in Griffintown. Opening would be in 2022. Around 200 apartments would be for seniors and 15 to 20 adapted apartments would be reserved for our project, situated on 1 or 2 floors. The majority of apartments would be 3 ½ and 4 would be 4 ½ (to be confirmed).
This project would aimed at adults with an ISO-SMAF profile of 4, 6, or 9 and who require between 25 to 45h of services per week.
Services would be ensured by the CIUSSS Centre-Sud -de-l’île- de- Montréal. The offer of services will be specified at a later date by the CIUSSS in collaboration with ÉquiToît. Services will include;

Individual needs for professional resources in home care(nursing, physiotherapy, psychosocial services,...) will be evaluated according to SAD criterias
